RN assessment and care-planning services may include:

We understand that staying connected to everyday life matters. Our caregivers can assist clients with getting to appointments, picking up essentials, and running necessary errands — providing safe, dependable support outside the home by arrangement.
Transportation & Errands may include:
- Transportation to medical appointments

Transportation services are provided by arrangement and based on caregiver availability, client needs, and Becky's Angels agency policy.

Short-term relief for family caregivers who need rest or time away.

Dementia & Alzheimer’s Care & Support
Becky’s Angels provides individualized in-home care and support for adults living with dementia, Alzheimer’s disease, and other forms of cognitive impairment. Care is focused on promoting safety, comfort, dignity, familiar routines, and the highest possible level of independence in the home.
Dementia and Alzheimer’s support may include:
- Assistance with bathing, dressing, grooming, toileting, and other activities of daily living
- Companionship and meaningful social interaction
- Gentle cueing, orientation, and redirection

Parkinson’s Care & Support
Becky’s Angels provides individualized in-home care and nursing support for adults living with Parkinson’s disease. Care is focused on promoting safety, comfort, independence, and consistency with daily routines while monitoring for changes that may need to be reported to the RN or healthcare provider.
Parkinson’s support may include:
- Assistance with walking, transfers, mobility, and positioning
- Fall-risk reduction and home safety support
- Assistance with bathing, dressing, grooming, toileting, and other activities of daily living
- Medication reminders and medication support within staff scope of practice and according to provider orders
- Observation for changes in mobility, tremors, stiffness, balance, cognition, swallowing, or functional ability
- Meal assistance and nutrition/hydration support
- Assistance with established exercise and activity routines as directed by the plan of care
- Skin integrity and pressure-injury prevention measures

- Assistance with activities of daily living, including bathing, dressing, grooming, and toileting
- Mobility, transferring, positioning, and ambulation assistance

Fall Prevention & Safety Checks
Becky’s Angels provides individualized in-home support to help reduce fall risks and promote a safer home environment. Fall-prevention measures are based on the client’s needs, functional abilities, RN assessment and individualized plan of care.
Services may include:
- Observation and identification of potential fall and safety risks
- Assistance with walking, transfers, and mobility
- Proper use of prescribed or recommended assistive devices
- Assistance with safe positioning and repositioning
- Monitoring for changes in strength, balance, mobility, or functional ability
- Assistance with toileting and other activities that may increase fall risk
- Keeping commonly used pathways and care areas free of hazards
- Encouraging appropriate footwear and other established safety measures
- Medication-related observations and reporting concerns to the nurse or provider as appropriate
- Client and caregiver education regarding fall-prevention strategies
- Documentation and reporting of falls, near-falls, injuries, and changes in condition
- Communication with the RN, provider, family, and other members of the healthcare team as appropriate
Important note: Fall-prevention and safety services are provided according to the client’s individualized assessment and plan of care, staff scope of practice and competency, provider orders when applicable, and Becky’s Angels policies and procedures.
Emergency situations or serious injuries require immediate emergency response, including calling 911 when appropriate.
Nurse-Led Skilled Support
Skilled Nursing Care
Becky’s Angels provides individualized skilled nursing services in the client’s home by appropriately licensed nursing professionals. Nursing care is based on the client’s assessed needs, individualized plan of care, applicable provider orders, nursing scope of practice, staff competency, and agency policies and procedures.
Personalized Nursing at Home
Our nurses work with clients, families, and healthcare providers to help carry out the established plan of care, monitor the client’s condition, provide ordered nursing interventions, and identify changes that may require additional evaluation or provider notification.
Important: All skilled nursing services are provided within the individual nurse’s professional scope of practice and demonstrated competency and according to the client’s individualized plan of care, applicable provider orders, and Becky’s Angels policies and procedures. Services are individualized, and the availability of a particular nursing service is determined following assessment of the client’s needs and the agency’s ability to safely meet those needs.
RN Assessments & Plan of Care
Becky’s Angels provides comprehensive Registered Nurse (RN) assessments to identify each client’s individual health needs, functional abilities, safety concerns, and care needs in the home. The assessment helps guide the development and ongoing review of an individualized plan of care.
RN assessment and care planning may include:
- Comprehensive initial nursing assessment
- Review of current health conditions and medical history
- Vital signs and general health status
- Medication review and reconciliation
- Pain and symptom assessment
- Skin integrity and wound-risk assessment
- Mobility, transfer, and fall-risk assessment
- Activities of daily living and personal-care needs
- Nutrition and hydration considerations
- Cognitive, behavioral, and communication needs
- Medical equipment and home-safety considerations
- Identification of skilled nursing and supportive-care needs
- Client and caregiver education needs
- Coordination and communication with healthcare providers

Individualized Plan of Care
Each client’s care is guided by an individualized plan of care based on assessment findings, identified needs, applicable provider orders, and the services authorized to be provided by Becky’s Angels.
The plan of care identifies the services to be provided, frequency and duration of care as applicable, client-specific interventions and safety considerations, and other information necessary to coordinate the client’s care.
The RN monitors the client’s response to care, communicates significant changes in condition to the appropriate healthcare provider, coordinates services, and reviews and updates the plan of care as required.
Important: Skilled nursing services are provided by appropriately licensed nursing personnel according to applicable provider orders, the individualized plan of care, professional scope of practice, staff competency, and Becky’s Angels policies and procedures.
Medication Support
Becky’s Angels provides individualized medication support to help clients safely follow their prescribed medication regimen at home. Services are provided according to the client’s assessed needs, individualized plan of care, applicable provider orders, staff role and scope of practice, demonstrated competency, and agency policy.
Medication Support may include:
- Medication reminders by appropriate staff
- Medication administration by licensed nursing staff as ordered
- Verification of medication orders prior to administration
- Oral, topical, inhaled, injectable, and other authorized medication administration within nursing scope and agency policy
- Observation and monitoring for medication effectiveness, side effects, adverse reactions, or changes in condition
- Medication reconciliation and review by the RN as appropriate
- Assistance with maintaining an accurate medication list
- Client and caregiver medication education
- Reinforcement of prescribed medication schedules and instructions
- Communication with the prescribing provider regarding medication concerns, discrepancies, adverse effects, or significant changes
- Documentation of medications administered in the client’s clinical record and Medication Administration Record (MAR), when applicable
- Safe medication handling, storage, and disposal education
Important: Becky’s Angels does not independently prescribe medications or initiate, discontinue, or change medication dosages or treatment regimens. Medication administration is performed by appropriately licensed nursing staff according to applicable provider orders, the individualized plan of care, scope of practice, demonstrated competency, and agency policy. Significant medication reactions or changes in condition are reported and escalated appropriately; suspected medical emergencies require immediate emergency response/911.
Professional wound monitoring and care support.
Includes dressing changes, infection prevention, healing observation, and documentation.
Becky's Angels provides skilled nursing support for clients with wounds or skin concerns that require care at home. Our licensed nurses may assist with wound observation, dressing changes as ordered, skin assessment, pressure injury prevention, infection monitoring, documentation, and communication with the family or provider if concerns arise.
Wound Care may include:
- Surgical wound care
- Pressure injury/bed sore support
- Dressing changes as ordered
- Wound measurement and observation
- Skin assessment and monitoring
- Pressure injury prevention
- Monitoring for redness, swelling, drainage, odor, pain, or signs of infection
- Wound VAC support, if ordered and within agency policy
- Drainage and dressing documentation
- Family education and communication
- Provider notification for concerning changes
Important note:
All wound care services are provided according to provider orders, nursing scope of practice, staff training, client assessment, and Becky's Angels agency policy. For uncontrolled bleeding, rapidly worsening wounds, fever, severe pain, or medical emergencies, families should call 911 immediately.
Becky’s Angels provides skilled nursing services for clients recovering at home following surgery. Licensed nursing staff provide services within their scope of practice and according to the client’s assessment, individualized plan of care, provider orders when required, and agency policies and procedures.
For the checklist, I would use:
- Post-operative nursing assessment and monitoring
- Vital sign monitoring
- Incision and skin assessment
- Dressing changes and wound care as ordered
- Drain care as ordered
- Pain and comfort assessment and monitoring

Important note:
All post-surgical nursing services are provided according to provider orders, nursing scope of practice, staff training, client assessment, and Becky's Angels agency policy. For severe pain, chest pain, uncontrolled bleeding, difficulty breathing, stroke symptoms, or medical emergencies, families should call 911 immediately.
Cardiac Care Support
Becky’s Angels provides skilled nursing support for clients with cardiac conditions or those recovering from a cardiac-related hospitalization or procedure. Care is individualized according to the client’s assessment, plan of care, provider orders when applicable, nursing scope of practice, and agency policy.
Cardiac Care Support may include:
- Heart rate, blood pressure, respiratory rate, and oxygen saturation monitoring
- Assessment and observation for changes in condition
- Monitoring for symptoms such as shortness of breath, swelling, dizziness, fatigue, chest discomfort, or changes in activity tolerance
- Medication assistance or administration as ordered and within staff scope
- Medication education and adherence support
- Monitoring for medication side effects or concerning changes
- Weight monitoring when indicated
- Observation for edema and fluid-retention concerns
- Diet and hydration education consistent with the established plan of care
- Mobility, activity, and fall-prevention support
- Post-hospital and post-cardiac-procedure nursing support
- Client and caregiver education regarding symptoms that should be reported
- Communication and coordination with the client’s physician, cardiology provider, and other members of the healthcare team
- Documentation and timely reporting of significant changes in condition
Important: Becky’s Angels does not replace the client’s cardiologist or other medical provider. Cardiac nursing services are provided within the nurse’s scope of practice, provider orders when required, the individualized plan of care, staff competency, and agency policy.
Chest pain, severe difficulty breathing, signs of stroke, loss of consciousness, or other suspected medical emergencies require immediate emergency response/911.
Diabetes Support
Becky’s Angels provides skilled nursing and supportive care for clients living with diabetes. Services are individualized according to the client’s assessment, plan of care, provider orders when applicable, nursing scope of practice, staff competency, and agency policy.
Diabetes Support may include:
- Blood glucose monitoring
- Continuous glucose monitor (CGM) observation and assistance, when applicable
- Insulin administration as ordered by the healthcare provider and within staff scope
- Administration of other prescribed injectable diabetes medications when ordered and within staff scope
- Medication assistance or administration according to provider orders
- Monitoring for signs and symptoms of hypoglycemia and hyperglycemia
- Observation and documentation of blood glucose trends
- Monitoring food and fluid intake when included in the plan of care
- Nutrition and meal-planning support consistent with provider recommendations
- Skin and foot observation and education
- Education regarding diabetes management and medication safety
- Reinforcement of the client’s prescribed diabetes care plan
- Communication with the healthcare provider regarding abnormal readings, symptoms, or changes in condition
- Documentation of blood glucose readings, medications administered, client response, and significant findings
Important: Diabetes-related nursing services are provided according to the client’s individualized plan of care, applicable provider orders, nursing scope of practice, staff training and competency, and Becky’s Angels agency policy. Significant changes in condition are reported promptly to the appropriate healthcare provider. Severe hypoglycemia, loss of consciousness, severe difficulty breathing, or another suspected medical emergency requires immediate emergency response/911.
Care for clients with breathing challenges.
Includes oxygen safety, respiratory monitoring, pulse checks, and symptom observation.
Becky's Angels provides skilled nursing support for clients with respiratory - related needs in the comfort of their home.
Our licensed nurses assess and monitor the client's respiratory status and provide nursing interventions within the nurses scope of practice, the client's individualized plan of care, provider orders, and Becky's Angels policies and procedures.
Nursing Care May Include:
- Respiratory status assessment and observation
- Respiratory rate, effort, and breathing - pattern monitoring
- Pulse oximetry monitoring
- Documentation of respiratory assessments, interventions, and client response
- Monitoring for shortness of breath or changes in respiratory condition
- Oxygen safety monitoring and assistance with prescribed oxygen use
- Administration or assistance with prescribed nebulizer treatments, as permitted within nursing scope
- Oxygen tubing and equipment safety checks
- Assistance with prescribed inhaled medications, as permitted within nursing scope
- Client and caregiver education prescribed care, oxygen safety, symptoms to report, and when to seek emergency assistance
- Communication with the client's provider regarding significant or concerning change in condition

These services are performed by appropriately licensed staff within their professional scope of practice and according to applicable provider orders, the individualized plan of care, staff competency, client assessment, and agency policy.
Becky's Angels does not represent these nursing services as respiratory therapy or services provided by a respiratory care practitioner.
Clients experiencing severe difficulty with breathing, respiratory distress, loss of consciousness, blue or gray discoloration, or another medical emergency should call 911 immediately.
Injections
Becky’s Angels provides skilled nursing services for clients who require prescribed injectable medications in the home. Injections are administered by appropriately licensed nursing staff according to provider orders, the individualized plan of care, medication-administration requirements, nursing scope of practice, demonstrated competency, and agency policy.
Injection services may include:
- Subcutaneous injections
- Intramuscular (IM) injections
- Prescribed insulin administration
- Other prescribed injectable medications within the nurse’s scope of practice and agency-approved services
- Verification of the medication order before administration
- Medication safety checks and appropriate injection-site selection
- Assessment and rotation of injection sites when applicable
- Monitoring for therapeutic response, side effects, or adverse reactions
- Blood glucose monitoring associated with ordered insulin therapy when applicable
- Safe handling and disposal of needles and other sharps
- Infection-prevention practices
- Client and caregiver education regarding prescribed injectable medications
- Communication with the prescribing provider regarding significant concerns or changes in condition
- Documentation of medication administration and the client’s response
Important: Injectable medications are administered only when supported by a valid provider order and the client’s individualized plan of care and when the medication and route are within the licensed nurse’s scope of practice and demonstrated competency. Becky’s Angels does not independently prescribe, initiate, discontinue, or change medication dosages. Suspected severe allergic reactions, difficulty breathing, loss of consciousness, or other medical emergencies require immediate emergency response/911.
Catheter Care
Becky’s Angels provides skilled nursing care and support for clients with prescribed urinary catheters. Services are individualized according to the client’s assessment, plan of care, applicable provider orders, nursing scope of practice, staff competency, and agency policy.
Catheter Care may include:
- Assessment and care of indwelling urinary (Foley) catheters
- Suprapubic catheter care and monitoring
- Catheter insertion, removal, or replacement when ordered, clinically appropriate, and within the nurse’s scope and demonstrated competency
- Routine catheter and perineal hygiene
- Catheter tubing and drainage-bag management
- Emptying and changing drainage bags as appropriate
- Monitoring and documenting urine output when indicated
- Observation of urine color, clarity, odor, and other characteristics
- Monitoring for leakage, obstruction, decreased output, discomfort, bleeding, or other complications

Important: Catheter services are provided according to the individualized plan of care, applicable provider orders, nursing scope of practice, and demonstrated staff competency. Becky’s Angels staff will promptly report significant changes or complications to the appropriate healthcare provider and follow established emergency procedures when indicated.
Ostomy Care
Becky’s Angels provides skilled nursing care and support for clients with established ostomies. Services are individualized according to the client’s assessment, plan of care, applicable provider orders, nursing scope of practice, staff competency, and agency policy.
Ostomy Care may include:
- Assessment and monitoring of established colostomies, ileostomies, and urostomies
- Stoma and surrounding skin assessment
- Routine stoma and peristomal skin care
- Ostomy pouch emptying and appliance changes
- Assistance with proper pouching-system fit and maintenance
- Monitoring the amount, color, consistency, and characteristics of output as appropriate
- Monitoring for leakage, skin irritation, breakdown, bleeding, swelling, or other changes
- Infection-prevention measures
- Monitoring hydration and nutrition concerns when indicated by the plan of care
- Client and caregiver education regarding ostomy care and appliance management
- Reinforcement of established ostomy-care instructions
- Identification and reporting of potential complications
- Communication and coordination with the healthcare provider or wound/ostomy specialist when appropriate
- Documentation of assessments, care provided, output when indicated, and client response
Important: Ostomy services are provided within the individual staff member’s authorized scope of practice and demonstrated competency and according to the client’s individualized plan of care and applicable provider orders. Significant changes or suspected complications are promptly reported to the appropriate healthcare provider, with emergency services contacted when medically necessary.
IV Therapy Support
Becky’s Angels provides skilled nursing support for clients who require certain prescribed IV therapies in the comfort of their home. IV services are provided by qualified licensed nursing staff according to provider orders, the individualized plan of care, nursing scope of practice, staff competency, and agency policy.
IV Therapy Support may include:
- Administration and monitoring of ordered basic IV fluids
- Administration and monitoring of ordered IV antibiotics
- Care and management of existing PICC lines, implanted ports, and central venous lines
- Assessment and monitoring of the vascular access site
- Dressing and site care as ordered
- Line flushing and maintenance according to orders and established protocol
- Monitoring for signs of infiltration, occlusion, infection, adverse reactions, or other complications
- Client and caregiver education regarding line safety and signs or symptoms that should be reported
- Communication with the prescribing provider and other members of the healthcare team
- Documentation of IV therapy, client response, and any changes or concerns
- Discontinuation of existing IV access by qualified nursing staff when ordered or clinically indicated and permitted by agency policy
Important: Becky’s Angels does not initiate, insert, or restart peripheral IV access. IV therapy is limited to services within the agency’s approved scope and the individual nurse’s scope, training, and demonstrated competency. Emergency symptoms or serious adverse reactions require immediate emergency response.
Tube Feeding Support
Becky’s Angels provides skilled nursing and supportive care for clients who receive prescribed nutrition, hydration, or medications through an established feeding tube. Services are provided according to the individualized plan of care, provider orders when applicable, nursing scope of practice, staff competency, and agency policy.
Tube Feeding Support may include:
- Care and monitoring of established G-tubes, PEG tubes, and J-tubes
- Administration of prescribed tube feedings by qualified staff
- Administration of prescribed water flushes
- Medication administration through an established feeding tube when ordered and within staff scope
- Feeding-tube site assessment and routine care
- Monitoring tube placement and function as appropriate before use
- Positioning during and after feedings to promote safety
- Monitoring feeding tolerance
- Observation for nausea, vomiting, abdominal discomfort or distention, diarrhea, constipation, leakage, or other changes
- Monitoring for signs of aspiration or other complications
- Infection-prevention measures and observation of the insertion site
- Intake and output monitoring when included in the plan of care
- Client and caregiver education regarding feeding-tube care and safety
- Communication with the provider regarding concerning findings or changes in condition
- Documentation of feedings, medications, flushes, tolerance, and nursing observations
Important: Tube-feeding services are provided only within the individual staff member’s authorized scope, training, and demonstrated competency and according to the client’s plan of care and applicable provider orders.
New feeding-tube placement and replacement of a dislodged tube are not represented as routine Becky’s Angels services. Suspected aspiration, respiratory distress, significant bleeding, or another medical emergency requires immediate emergency response.
Skilled nursing care for ordered drainage devices may include Jackson Pratt drains, Hemovac drains, Penrose drains, nephrostomy tubes, biliary drains, pigtail drain. Care may include assessment, site care, dressing changes, monitoring drainage characteristics and output, maintaining device safety, client/caregiver education, documentation, and reporting changes or concerns to the appropriate healthcare provider.
Services are provided only when included in the individualized plan of care and supported by applicable provider orders, and are performed by appropriately licensed nursing staff within their scope of practice and demonstrated competency, in accordance with Becky’s Angels policies and procedures.
